The way it works in the US stinks! Same for Canada (maybe)! How do we fix it?

I just got a letter from the hospital saying that we owe the $200 copay and not the $50 copay because Punk's ER visit wasn't an emergency.  I guess our family doctor telling him his retina might be detaching is just a minor inconvenience.

i got the same thing feffer. i went to the er and had to be given 2 bottles of fluids and other remedies to keep me alive. my HR director said i needed to call and dispute it and they would more than likely change it. it was just SOP to classify everything less than decapitation a non-emergency.

yeah, dying is just a minor inconvenience to the health "care" system.
you'd be surprised how much a nutsack can stretch. you have to stretch it yourself, not a woman. they don't do it quite right.

Canada's system needs work, but compared to the US system, I'm thrilled. People here complain but they have no basis for comparison in many cases. I find plenty of USians willing to tell me how much "socialist" health care sucks, but I wouldn't trade places. Don't give me that "higher taxes" crap either, my standard of living is the same or better here than it was in the US, and I have the bonus of knowing if the shit hit the fan we wouldn't go bankrupt.

I'll offer a point for change - here, much as in the US, vision and dental are considered supplemental, an "extra" that you have to get through work insurance, and quite limited at that. What makes eyes and teeth less important than the rest of your body??

Canada has great reserves of doctors in the tar sands of northern Alberta. If they need more they just drill baby drill.

Yes, we've had a brain drain. They could fix that by figuring out how to efficiently certify skilled immigrants though. We have doctors from other countries driving taxis and cleaning office buildings, which is a stupid shame.

Also, the Conservatives fuck over health care whenever they get in power. They could pay doctors more, and make it less of a revolving door in the clinics. Still, this is no less than what most US health care provides except OH WAIT FOR IT it's free to us and just as competent.

And they do some things very right that the US does not do - for instance midwifery care is absolutely free in our province even if you're not a landed immigrant/citizen. The midwives take on many of the low-risk cases providing homebirth options in addition to hospital deliveries, which lessens the burden on the OB/GYNs in number of patients that don't need to be under a doctor's care and the burden on hospitals for having to provide services for low-risk deliveries. In the end it saves money and it's less hassle on everyone with bonus to the patient of extra involved care, win-win all around.

Let me redirect back to the original question "how do we fix it?" which is more productive than "snark on the country next door to you that *doesn't* charge for emergency room visits".

They already refused to pay for the second sonogram, because, you know, finding out what's wrong is totally unnecessary.

The usual policy answer is that there's less formal distinction, in those regions of medicine, between necessary and elective procedures.  In other words, corrected vision may be a social (in the larger sense of 'social') necessity, but are designer glasses?  Contact lenses?  The Army used to issue standardized cheap-ass black-plastic-frame eyeglasses, period.  If public health care did the same, would people who couldn't afford to upgrade feel stigmatized?  It would be an outward sign of poverty, and god knows if you don't have money to spend on insanely overpriced designer frames, then there must be something genetically wrong with you.

Ditto for toothcare.  Cavities and tooth loss are against the public interest; that's easily demonstrated.  What about braces?  Caps?  Governments have a hard time settling these kinds of questions, and trade groups (ie, eye doctors and dentists) have conflicts of interest.  Private insurance companies offer spiffier plans to entice customers, not because they think it's what's necessary or good -- that's why cosmetic dentistry is often covered but therapeutic psychology often basically isn't.  People are happy to admit they want that California Smile but are less apt to admit they want therapy.  We're a nation of idiots.

I'm not saying these problems are actually so hard to solve, but they're very hard for politicians.

Personally, of course, I prefer socialized medicine.  I think it's a no-brainer.  Places where it's been given a proper try in the US almost always show a sharp reduction in tax liability even in up-front costs, and it's patently in society's favor.  But, then, I can't afford fancy medical care, myself.  Even a three-month wait for some free procedure would beat never having it.

I don't understand the dental insurance issue myself, considering that in the last few years studies have shown that poor dental hygiene leads to a host of al manner of bad health issues.

To me, it seems like gray area procedures like caps or braces can just as well be given some pre-determined coverage limit, and you or your supplemental insurance from your employer would shell out the remaining balance. You can argue that braces and caps might be "vanity" procedures, but they too have advantages with dental health. Like if you're someone who has very thin or soft tooth enamel, caps keep your teeth from disintegrating.

As for vision insurance, IIRC the yearly cost of disposable contacts was about equivalent to buying a new pair of glasses every year, which seems to be about the average time I have glasses before I break them or my vision changes.

I still don't get why corrective vision is not better covered by insurance. I'd go so far as to say vision coverage is preventative -- i.e. prevents the borderline blind from crashing their cars and running over people.
